Overview
Bunion Pain
A bunion is a prominence over the inside part of the foot where the big toe joins the rest of the foot. Pressure on the prominence from shoes causes pain and swelling due to inflammation. The bunion occurs when the foot bone connecting to the big toe (the first metatarsal) moves gradually towards the opposite foot. This is called hallux valgus deformity. This leads to the big toe being pushed towards the second toe (away from the opposite foot) so that the big toe points away from the other foot. Other problems can develop with a bunion. For example the second toe may overlap the big toe causing a cross-over toe deformity. With shoe pressure, corns and calluses develop.

Causes
Bunions are most often caused by faulty mechanics of the foot. The deformity runs in families, but it is the foot type that is hereditary, not the bunion.Certain foot types make a person prone to developing a bunion. Injury can be a cause, especially if just one foot is involved. Although wearing shoes that crowd the toes won?t actually cause bunions in the first place, it can make the deformity progressively worse. That means you may expererince symptoms sooner.
SymptomsThe signs and symptoms of a bunion include a bulging bump on the outside of the base of your big toe, swelling, redness or soreness around your big toe joint, Thickening of the skin at the base of your big toe, Corns or calluses, these often develop where the first and second toes overlap, persistent or intermittent pain, restricted movement of your big toe. Although bunions often require no medical treatment, see your doctor or a doctor who specializes in treating foot disorders (podiatrist or orthopedic foot specialist) if you have persistent big toe or foot pain, a visible bump on your big toe joint, decreased movement of your big toe or foot, difficulty finding shoes that fit properly because of a bunion.

Diagnosis
Diagnosis begins with a careful history and physical examination by your doctor. This will usually include a discussion about shoe wear and the importance of shoes in the development and treatment of the condition. X-rays will probably be suggested. This allows your doctor to measure several important angles made by the bones of the feet to help determine the appropriate treatment.

Non Surgical Treatment
Depending on how many of the causative factors are true, a series of exercises to ensure correct alignment and stability of the lower limb should be implemented. Supportive foot wear with correct width and arch support can provide relief -shoes such as ballet flats, thongs (flip flops) and Ugg boots (or slippers) should be avoided. Mobilization of the mid foot to help re-align the toe correctly, and then taping and padding in the shoe to keep the toe in alignment. Taping to help draw the 1st metatarsal back in towards the second and correct any rotation and drop of the 1st metatarsal. Foam padding shaped like a donut to off load the pressure on the outside of the big toe.

Bunion surgery is both an art and a science, and it should always be performed by a healthcare professional who understands the multifaceted mechanics of your first MTP and its surrounding structures. Bunion surgery involves more than just removing the bump at the base of your big toe, and it is best followed up with supportive therapies to prevent a recurrence of your health problem.
